How it Works

Midwest Health Monitoring works with physicians to set up RPM services for the patient. The process is fast and easy- requiring only a referral to get started.

Step 1

Identify Eligible Patients

We’ll start by helping you identify the patients in your practice who are eligible and recommending the device that will best serve their diagnosis.

Step 2

Patient Onboarding

The Midwest Health Monitoring team then takes care of the admission process for each patient. You don’t have to worry about anything! We’ll reach out to the qualifying patient to explain the program, its benefits, and any financial responsibility that may fall on them. We’ll then register the appropriate device and deliver it to the patient’s home–and we’ll document everything.

Step 3

Patient Monitoring

We’ll monitor patients’ physiological data daily. The RPM device will notify Midwest Health Monitoring when a patient’s data has exceeded the established parameters, and Midwest Health Monitoring will notify you and, ideally, trigger an in-office visit that allows you to make the best clinical decision for your patient.

Relax

From the first step of identifying qualifying patients through to billing, we’ve fine-tuned our process to minimize the hassles and headaches.
