Midwest Health Monitoring works alongside your doctor to watch over your health between appointments, so small changes are caught early — before they become big problems.
Remote Patient Monitoring (RPM) is a way for your doctor to keep an eye on your health from home — no extra office visits required.
When you join the program, you’ll receive a small device — such as a blood pressure cuff, a scale, a pulse oximeter, or a glucose meter — that fits easily into your daily routine. The device automatically sends your readings to a secure platform where our clinical team reviews them every day.
If anything looks concerning, we reach out to you — and if needed, we contact your doctor right away. Think of it as having a caring, watchful health coach available to you every single day.
The program is designed for people managing ongoing health conditions, with just a few examples being:
Your doctor’s office will let you know if you are a good candidate for the program.
These are real examples from our program. Patient details have been kept general to protect privacy.
Our monitoring team noticed that a patient's blood pressure was dropping dangerously low at certain times of day. After investigation, it was found that the timing of their medication was causing the problem. The physician adjusted the schedule — and the patient avoided a serious fall or fainting episode.
One of our patients' family members shared that knowing someone was watching over their loved one's health every day gave the whole family peace of mind. "We don't worry as much anymore," they said. "We know someone is paying attention."
A patient's heart rate readings raised a concern during routine monitoring. Our triage nurse reached out, gathered information, and the patient was seen by their physician the same day — where they were diagnosed with atrial flutter and scheduled for cardioversion. Early detection made all the difference.
No. The devices we provide use built-in cellular technology to send your readings automatically. You do not need a smartphone, a computer, Wi-Fi, or Bluetooth. If you can use a blood pressure cuff, you can use our devices.
Life happens — and missing a day is not a crisis. If we notice you have not taken a reading in a few days, your health coach may reach out to check in and make sure everything is okay.
Your readings are shared with your doctor and with our trained clinical team, who review them on your behalf. Your data is kept secure and private in accordance with HIPAA regulations. It is never sold or shared for advertising purposes.
Real people review your readings — our health coaches and registered nurses. If something warrants a call, a real person will reach out to you personally. You are never just a number in a computer system.
Yes. Participation is always voluntary. If you decide the program is not right for you, simply let us know and we will take care of everything, including returning or stopping the device.
The key difference is that with RPM, your readings do not just sit in a notebook — they are reviewed by trained clinical professionals every single day. Patterns are analyzed over weeks and months. And when something looks wrong, someone who knows what to look for acts. Self-monitoring is a great habit, but RPM adds a layer of expert oversight and a direct connection to your care team that makes all the difference.
If your doctor has recommended remote patient monitoring, or if you’d like to learn whether you qualify, reach out to us. We’d be happy to answer your questions and walk you through what to expect.

A patient in our heart failure program gained six pounds in a single day — a warning sign of dangerous fluid buildup. Our health coach spotted the change immediately and notified the patient's physician team. The patient was given their prescribed diuretic promptly, their fluid balance improved, and a hospitalization was avoided entirely.
