For Patients

Your Health, Monitored Every Day — Not Just at Office Visits

Midwest Health Monitoring works alongside your doctor to watch over your health between appointments, so small changes are caught early — before they become big problems.

What Is Remote Patient Monitoring?

Remote Patient Monitoring (RPM) is a way for your doctor to keep an eye on your health from home — no extra office visits required.

When you join the program, you’ll receive a small device — such as a blood pressure cuff, a scale, a pulse oximeter, or a glucose meter — that fits easily into your daily routine. The device automatically sends your readings to a secure platform where our clinical team reviews them every day.

If anything looks concerning, we reach out to you — and if needed, we contact your doctor right away. Think of it as having a caring, watchful health coach available to you every single day.

Who Is This Program For?

The program is designed for people managing ongoing health conditions, with just a few examples being:

  • High blood pressure (Hypertension) —Daily monitoring gives your doctor a much clearer picture than a single reading at an office visit, leading to better medication decisions and steadier control.
  • Heart failure (Congestive HeartFailure) — Daily weight readings help catch dangerous fluid buildup days before symptoms become serious, often preventing a hospital stay.
  • Diabetes —Regular glucose readings help you and your care team spot patterns, adjust treatment, and avoid dangerous highs and lows.

Your doctor’s office will let you know if you are a good candidate for the program.

Real Stories From Real Patients

Your Results — What Our Patients Experience

  • 83% of patients with high blood pressure showed measurable improvement in their readings during the monitoring period.
  • 63% reached their blood pressure target range — a goal that often takes years to achieve with standard office-based care alone.
  • For patients with heart failure, the number of dangerous rapid weight-gain events fell by more than 50% within three months.
  • For patients with diabetes, dangerous blood sugar events — both high and low — dropped by 76% within three months.
  • Real-time monitoring helped our clinical team intervene before an ER visit or hospitalization was needed, on multiple documented occasions.

These are not just statistics. Each number represents a person who avoided a frightening health scare, stayed out of the hospital, and got to remain comfortably at home with their family.
