Our Implementation
We gather eligible patients and provide a financial analysis.
The list of eligible patients is approved by the practice.
Patients are mailed an approved letter with practice letterhead.
Our clinicians contact the patient, explain the program and review any financial obligation if any.
Once patient consents, they are mailed the devices based on diagnosis. Devices are tracked and patients are called and brought through how to properly use the device and answer any questions.
The program is commenced, a health coach assigned, and patient begins monitoring.
